By completing the data collection worksheets, your learning institution will help your students receive federal funding to complete their education. Please keep in mind that you may receive requests from our two different programs, the National Health Service Corps and NURSE Corps; you will complete the actions in the same place. So now let's take a look at how to complete your school's Data Collection Worksheets. You'll first need to log in to the website: program portal.RSA.gov and select that you are a School Representative. Here, you'll log in with your portal username and password. If you don't have one, would need to click 'create a school representative' account to set one up. If you try to create an account and receive the message, 'The email address provided is not found in our database,' you will need to contact the Customer Care Center at 1-800-221-9393 to resolve the issue. If you choose to create a new account, you'll receive the following message, and you'll receive an email with a link to activate the account. Once you're logged in, you'll be directed to your school's homepage. Once on your school's homepage, you'll see all of your contact information. If any of your information needs to be updated, you can do it here and click the 'save' button. If you don't need to update anything, under the 'save' button you will find the schools associated with your account. Select the school and complete the Data Collection Worksheet for it. Now you'll find yourself on the school screen and at the top you'll see what information we have on file as being your EIN and DUNS numbers. If these are correct, no further action is needed, and you can proceed. If the field is blank, please contact either the National Health Service Corps or NURSE Corps with the correct information. Next, we'll take a look at the Data Collection Worksheets that need to be completed based on what disciplines and degrees that your school offers.
